If a visually impaired person has passed Class 10 and wishes to apply for a smartphone, the following documents may be required: UDID Card or Disability Certificate (photocopy) Medical Certificate (photocopy) Aadhaar Card (photocopy) Caste Certificate, if applicable (photocopy) Residence/Domicile Certificate (photocopy) Class 10 Marksheet or Passing Certificate (photocopy) Four passport-size photographs Income Certificate (photocopy) If the visually impaired applicant does not have personal income and is dependent on family members, the income certificate of a parent or guardian may be accepted, depending on the scheme requirements. Applicants should ensure that all documents are complete and up to date before submitting their application.
