If you want to take a legal and proper approach, then directly visit your district hospital, get an eye test (or relevant medical test for your disability), and request the CMO or specialist doctor for your certificate. Say clearly: “Sir, please issue a 100% disability certificate for me.” They will prepare your certificate, and based on that, your UDID card will also be created.
