Why is this important? Because when it comes to people with disabilities (divyang), you will be asked for a Disability Certificate. You had also asked what a Disability Certificate is. So let me explain: A Disability Certificate is an official document that certifies a person’s disability. It is issued by the District Medical Board. Nowadays, a UDID card (Unique Disability ID) is also issued, which is digital and recognized by government offices. When you visit the official service center, carry these documents: Your UDID card or Disability Certificate (issued by the District Medical Board). Your Aadhaar Card. At the service center: They will take your photograph. Your documents will be uploaded. You will be verified and the SIM will be issued or ported in your name. Please note: This process usually cannot be done at small retail shops or street vendors who sell SIM cards, because they do not have the facility to process documents for people with disabilities.
